This case study demonstrates how technology is being used to activate patients to participate in their own health and care in cardiovascular services. The use of patient management tools with functionality to support the management of patients at risk of stroke. Using an application hosted on the NHS spine, via N3, and designed to be interoperable with a wide range of hospital and GP systems. It is accessible via a web browser on desktops, tablets and mobile devices.
