Chronic diseases are now the most common cause of death and disability in England. More than 15 million people have a long-term condition such as hypertension, depression, asthma, diabetes, coronary heart disease, chronic kidney disease, or other health problems, for which there is no cure. This group tend to be heavy users of the health service, accounting for at least 70 percent of all NHS spend.
